Join Our Study (GP Collaborators)

• GP Collaborators are expected to:

1. Complete a clinical collaboration agreement

2. Complete the training provided by LKCMedicine: CITI certification, training video, site visit

3. Recruit 15 patient participants within 6 months or less

4. Adhere to their randomly assigned research arm and its intervention procedures

5. Conduct a post-intervention health screening per patient participant
